Hospital readmission rates are a true quality indicator of a hospital system, indirectly linked with communication, coordination, quality of care and collaboration.
Hospitals and health systems are experiencing avoidable costs where there are high inappropriate readmission rates. 27% of readmissions are potentially avoidable and there are increasing levels of acute care and/or readmissions where expenses exceed funding.
Reducing unplanned avoidable readmissions is a key quality issue and requires a hospital wide strategy for improving systems for all patients.
