Hospital readmission rates are a true quality indicator of a hospital system, indirectly linked with communication, coordination, quality of care and collaboration.

Hospitals and health systems are experiencing avoidable costs where there are high inappropriate readmission rates. 27% of readmissions are potentially avoidable and there are increasing levels of acute care and/or readmissions where expenses exceed funding.

Reducing unplanned avoidable readmissions is a key quality issue and requires a hospital wide strategy for improving systems for all patients.

Frequent hospital admissions can be a sign that patients with chronic conditions are not provided with adequate access to integrated care.

The 2020 Readmission Improvement Group will continue the successful progress made in previous years by:

2. Developing specific Readmission benchmarking reports that focus on quality, safe and efficient patient care
3. Measuring and benchmarking readmission performance including demographic, specialty and diagnosis level
4. Analysing the reports to identify unique insights for your organisation
5. Connecting with highly regarded Readmission experts on key topics to improve your patients discharge
6. Sharing and collaborating best practice models of care from your organisation

 

The Health Roundtable has developed detailed benchmarking reports to assist services to highlight key insights and review the success and opportunities in their readmission rates against peers.
