In 2014 we are holding a special Roundtable to improve how   hospitals and primary care organisations work together to provide coordinated care for patients with chronic conditions.

Many members in New Zealand and Australia are already working closely with primary care organisations, but recognise that more needs to be done to improve patient care.  This meeting will help you to compare your practices with other leading organisations and to identify further improvements.
