The Medical Patient Journey Improvement Group is now in its second year and, brings together the previous Chronic Disease and Readmissions improvement groups. The theme for the 2019 workshop was chosen by members is Outcome-based models of care for complex patients – exploring best practice for the bariatric and frail patients.


The benchmarking report for this group aims to consider the patient journey, and focuses on patients with COPD, stroke, diabetes, chronic heart failure and dementia. In 2019 outcomes related to back pain are introduced in the report. The Health Roundtable, identified a cohort of frail patients within the inpatient data for the first time in 2018. The frailty indicator received positive feedback from attendees, who valued the additional insights regarding frail patients within the hospital setting that the report provided. In 2019 the medical patient journey report will continue to review this frail patient cohort.

Key insights and general observations from the first Medical Patient Journey Improvement Group benchmarking report. Interactive discussion throughout report as key areas are highlighted and common themes emerge.

At the end of the session, attendees who are interested in understanding how the benchmarking data is compiled and how it can be used, are invited to meet with members of health roundtable team who will be available over morning tea to share how to access the information and use the reports and answer any data questions.

Small group discussion based on bariatric, frailty or other common themes, with other participants – identifying key issues, how they can be addressed, connecting with and learning from peers.

 

Discussion guide to facilitate conversation will be provided.

Jumping Obstacles - Activity using service design thinking principles to identify and mitigate potential challenges implementing a change. Provide attendees with actionable strategies on how to address challenges faced when returning to the workplace and seeking to action innovations and service improvements.

COMMITMENT

Participants will work with their own health service team to develop their Aim Statement and Action Plan based on the insights, innovations and shared learnings. Using worksheet and templates provided.
