Alfred Health Develops GEM at Home Program to Reduce Acute Admissions and Improve Patient Experience

Alfred Health and Caulfield Hospital wanted to reduce admissions to Subacute or Acute beds and provide a home based solution to meet geriatric patient’s needs.

To address achieve their goals Alfred developed a GEM at Home Program. Characteristics of the program include:
• Admitted substitution model
• 7/wk service with 24 hr telephone support
• Goal orientated and time limited
• Daily visits from 1 or more disciplines
• Capacity to flex bed numbers (avg 11 beds)

Within the first year there were 286 admissions to the GEM at home program with 45% coming direct from the acute setting and a 9% originating from the community.
