West Moreton Uses Daily Safety Briefing to Improve Communication and Avert Serious Events

Communication was siloed and a lack of clarity around accountability for the detection, prediction and management of safety and quality risks were a barrier to patient safety.

A Daily Safety Briefing (adopted from the Cincinnati Children’s Hospital and based on high risk but high reliability industries such as aviation and nuclear energy) was adopted. The Daily Safety Briefing is led by an Executive and is part of a three tier risk identification and management structure. It requires 20 minutes daily for a tightly structured briefing that includes:
•Look back – unpredicted events
•Current State – risks and issues allocated green, yellow, or red
•Look ahead 24/48hrs – highlight the predicted future state and follow-up

West Moreton has found that the Daily Safety Briefing has assisted in increasing the visibility of issues across the organisation and allowing for quicker responses to issues.
