At the Patient Safety Improvement Workshop, 19-20 Oct, Raj Behal, Chief Quality Officer, Stanford, challenged the group to consider if their culture had the following dysfunctional features: ignoring the rules, not talking about difficult issues, blaming individuals, normalising deviance, not learning from problems, no aspirations for safety goals

Culture alone does not cause harm – it is the complex interactions among multiple contributory factors, including local norms – that cause harm

The role of leaders in culture is critical- staff notice what leaders pay systematic attention to: What they ask about, measure, reward, fix or ignore, allocate resources and what gets ENDURING attention. To view this presentation login and  click here
