Whanganui Revises their Diabetes Service Delivery Model to Improve Screening Rates

 Whanganui DHB was ranked 21st nationally out of 21 DHBs in New Zealand for Long Term Conditions set targets for diabetes and cardiovascular disease.

In response to the above, Whanganui developed a patient centric model of care that promotes highly connected networks of professionals and supporting infrastructure that:
• Introduced clinical decision making support tools
• Sustained partnership planning and active community participation in health improvement initiatives
• Developed work force with practice nurse training

Since formal introduction of the new model of care Whanganui DHB is currently ranked 2nd nationally with 91% and exceeds the target of 90%.
