At Nepean Hospital the HITH program is required to cover a large geographical area, lacked continuity of care and had poor access to in-hospital medical records which required carrying large volumes of patient notes to visits.
As a result, Nepean developed a mobile App that:
• Keeps photographic documentation of wounds/cutaneous lesions etc.
• Has an external link to hospital medical records
• Supports RN progress notes and uploading to the central database
• Supports videoconferencing from patients’ homes should nurses need a second opinion
Since implementing the App:
• 1005 patient details have been entered onto the system; 42 staff members are registered users
• Record keeping has been streamlined and the need for staff to carry paper notes has been reduced
