Royal Melbourne Hospital Implements a Person-Centred Care Bundle to Improve Care

At Royal Melbourne Hospital (RMH) there was significant variation by ward in the approach to providing safe and person centred care. Furthermore, patient experience data highlighted that RMH needed to do better at engaging patients in their care.

To improve their care RMH:
• Implemented a Person Centred Care Bundle with 5 critical strategies launched:
• Nursing Assessment and Care Planning
• Bedside Handover
• Patient Safety Rounding
• Patient Whiteboards
• Safety Huddles
• Implemented Patient Experience Applications

As a result RMH has noted the following improvements:
• Increased Recommender Score from 80 to 85
• Decreased Falls Rate
• Decreased Pressure Injury Rate
• Increased Hand Hygiene Rate
