Modbury Hospital Uses TOP 5 Tool to Improve Care by Engaging Consumers and Carers

Modbury Hospital was experiencing an increase in patients with dementia/delirium, the number of code blacks and utilization of chemical restraints.

As a result, Modbury implemented the TOP 5 Tool that was first developed by NSW Health. The tool engages clinical staff in a structured process with carers to record up to five important non-clinical ‘tips’ and management strategies to aid communication and support personalized care.**

Since implemetnign the tool a reduction in complaints (and in increase in compliments) and a reduction in challenging behaviour incidents has been achieved.
