Logan Hospital Transitions towards to a 7 day clinical pharmacy service

At Logan Hospital pharmacy workload on the weekends was increasing; dispensary based pharmacists were busier and Monday ward pharmacists were having large amounts of histories, reconciliations, reviews and discharges to facilitate.

In an effort to improve performance a 7 day floating pharmacy roster providing 50% service level over the weekend was introduced (aim was to mirror weekday services).

Since implementing the interventions in 2013 improvements in timeliness of medication review has resulted. However, pharmacist intervention data appears to be dropping since the implementation of the ED service; this requires further investigation. Further work to understand the impact of weekend service on patient outcomes is also planned.
