Royal Brisbane & Women's Hospital Develops Decision Support Tool for ED Transfers from RACF

Royal Brisbane & Women's Hospital received ~120 acute presentations in ED from RACFs per month with ~30% being returned to RACF direct from ED. It was determined that patients could be more appropriately managed; some within the RACF.

To better manage RACF patients a decision support pathway inclusive for all RACF patients was developed. The Hospital in the Nursing Home (HINH) team uses the tool and liaises between RACF and ED.

Currently the Decision Support tool is being trialed in two RACFs with broader implementation planned for the future. Participants are aiming for better communication between RACF and HINH and an increase in the number of residents who are able to remain in RACFs.
