Logan Reduces ED Visits and Readmits with Chronic Disease Integrated Nurse Practitioner Care Model

Logan Hospital was experiencing long wait times for chronic disease specialist appointments and was also relying too much on the ED for suboptimal care. To remedy the above issues a chronic disease nurse practitioner clinic was established.

The clinic offers comprehensive patient assessments, exercise/lifestyle programs, coordinated approach to healthcare and telephone follow-up.

Since implementing the clinic in June of 2014 96% patients attended their scheduled appointments, 78% had no Emergency Department visits related to their chronic disease and only 5% required hospital admission.
