Monash Health implements a “Discharge Passport” to support patients returning home

At Monash Health’s Kingston Sub Acute Services, a review found that there was no formal information given to patients regarding follow up or contact numbers on discharge. They implemented the “Discharge Passport” to improve patient experience and ensure safe discharge to appropriate ongoing services.
