Caulfield Hospital finds a timely and effective way to transfer patients from acute to sub-acute

With a history of lengthy clinical handover from acute to sub-acute and a low rate of discharge summaries completed, Caulfield Hospital decided to use this as an opportunity for positive change. They implemented a “direct” pathway for all patients to transfer to subacute whereby the decision to admit rests entirely with the acute team. They also mandated a medical clinical handover to occur prior to handover.
 
Patient wait times have now dropped, discharge summaries have risen from 40% to 100%, there has been no increase in clinical incidents as a result and trust and confidence between clinicians has grown.
