At Alfred Health the Pharmacists are reducing medication errors

A randomised trial of credentialed pharmacists taking medication histories and charting medications on admission has resulted in impressive results. The rate of identified medication errors was 0.5% in the pharmacist arm compared to 35.3% in the medical arm.
