Calvary Mater Newcastle have reduced their readmissions to one of the lowest of HRT Members.

Calvary Mater Newcastle have reduced their readmissions and now have one of the lowest rates of medical readmissions especially for people aged over 80 years. Importantly they have undertaken a lot of work understanding WHY their patients are returning to hospital,  particularly the socio-economic factors that contribute to a patients’ readmission.

The Hunter Alliance formed with the Medicare local and the LHD, are working to prevent the need for hospitalisation by rapid access to expert advice including health pathways, information for patients and developing special support programs for COPD, diabetes and those patients likely to be in the last 12 months of life.
