Please register your delegates by 21 May 2010 and indicate which Workshop each delegate will be attending on Day 2 – 4 June including the Dinner (optional) on 3 June.

Delegate Registrations may be cancelled up to 27 May 2010, with a full credit of monies paid. After that date, no credits are possible. However, you may substitute a different person for those registered at any time. Hotel and airfare arrangements are separate, and may have different policies regarding cancellations and name changes. Please contact them directly.

  • Overview Presentation of KPIs by domain.
  • Discussion of variations.
  • Discussion of Data Issues, Definitions & Collection Methodologies.

Review of Community KPIs extracted from Raw Data:
  • Discussion of variations.

  • Planning for the next round of community data collection – including further analysis.

Progress on operational improvement initiatives from August Meeting 2009:
  • Health services to provide an update on progress towards the aim statements they generated at the August meeting.
  • Brief 10 minute PPT presentations by each team demonstrating outcomes.

Patient Flows across the continuum (to non-government organisations):
Each team to provide a brief poster presentation on strategies they have used to improve:
  • Flow of clients.
  • Getting placements.
  • Sharing of information.
  • Referral processes.
  • Continuity of care.
  • Working with NGOs.

Triage Models:
Each team to prepare a 10 minute presentation of innovations in triaging patients and triage models (e.g. call centres, streaming of clients).

Seven Day Follow-up post Discharge:
Each team to prepare a brief poster presentation of key strategies and innovations in managing the critical seven day follow-up post discharge (e.g. methods of follow-up, managing the hard to follow-up clients).

Hot Issues Forum:
Participants to raise and discuss their hot issues in regards to running their service and have opportunities for others to provide input.

Accommodation & Travel/Flight Arrangements
 
Delegates are responsible for making their own accommodation and travel/flight arrangements.
 
The Novotel Brighton Beach has set aside rooms for the nights of 2 and 3 June 2010 for The Health Roundtable at the Conference rate of AUD$199.00 (Standard King) – room only including GST or $219.00 inclusive of 1 x full buffet breakfast in the Baygarden Restaurant.
