Please register your delegates by 25 September 2009. Delegate Registrations may be cancelled up 8 October 2009, with a full credit of monies paid. After that date, no credits are possible. However, you may substitute a different person for those registered at any time. Hotel and airfare arrangements are separate, and may have different policies regarding cancellations and name changes. 9.00-10.30am	Alternative stream for SAFE teams: Development of Lessons Learned and Keys for Success (SAFE teams can attend either session) (a) What should HRT do differently next time – for new teams? (b) What (if any) ongoing support is needed for current teams?

11.15-11.45am	Feedback from SAFE teams: Group discussion: How to get more consistent reductions in inpatient mortality

Accommodation & Flight Arrangements

Delegates are responsible for making their own accommodation and travel/flight arrangements.

The Sofitel Gold Coast has set aside rooms for the nights of 13, 14 and 15 October 2009 for The Health Roundtable at the Conference rate of AUD$220.00 Classic River View Room and AUD$240.00 Superior Ocean View Room (room only including GST).

To receive this rate, Delegates are required to complete and return the attached Accommodation Booking Form to the Hotel as soon as possible.
