NOTE: If those from your DHB who are attending on the Friday want to attend the Executive Briefing session with Dr Ken Catchpole at 3.30pm on the Thursday they can register. There is no charge for this session only.

Delegate Registrations may be cancelled up 10 September 2009, with a full credit of monies paid. After that date, no credits are possible. However, you may substitute a different person for those registered at any time. Hotel and airfare arrangements are separate, and may have different policies regarding cancellations and name changes. Please contact them directly.

Executive Briefing: Dr Ken Catchpole 
In a 90-minute briefing, Ken will discuss his work with the Ferrari Formula 1 racing team to apply pit-stop principles to patient handovers from surgery to intensive care, which was short-listed for the Times Research Project of the Year in 2007, and has been adopted internationally by a variety of hospitals and quality improvement organisations.

Workshop: Identifying and Reducing Errors in Surgical Care (Dr Ken Catchpole)
Ken will lead an interactive workshop to discuss ways to reduce adverse events in clinical practice. He will highlight how to use empirical research methods to make evidence-based changes in practice, by taking the time to experience, observe and discuss the causes at the coalface.
Ken will share his experience from several studies including an examination of the mechanism by which errors in surgery cause harm, an empirical evaluation of aviation-style training in teamwork to reduce surgical error, and a project to reduce adverse incidents on surgical wards using Toyota production principles.
