Data Presentation: Medical Patient Journey Report
We would like to invite your Health service to participate in our 2020 Medical Patient Journey Improvement Group webinar on April the 29th this will be a 1-hour review of the Medical patient journey reports.
We are hoping that we will be able to deliver the planned keynote address latter this year and will communicate with members when details are confirmed.
The Medical Patient Journey Improvement Group concentrations on patients with chronic diseases and multiply complexities. The theme for 2020 is Improving the delirium patient journey: putting words into action.
The benchmarking report for this group aims to consider the patient journey, and focuses on patients with COPD, stroke, diabetes, chronic heart failure, dementia and frailty. In 2019 outcomes related to back pain were introduced in the report.
The goals of the group are to:

Improve care for patients with chronic conditions

Reduce unplanned avoidable readmission

Identify “exemplar performers” coordination of care for chronic diseases

Learn how the exemplar hospitals are working together with primary care providers and improve coordination with your primary care service

Share innovations in practices with an aim to improve patient experience and outcomes
