The Integrated Care Special Interest Group was established to address the growing need in health care — improving care delivered across the continuum of care - especially where the community and hospital settings intersect.

This group is an extension of the work started in 2017 & 2018 with the New Zealand Whole of System Program and in 2019 now includes both Australian and New Zealand health service members.

We know that providing integrated care is challenging; as it occurs when the work of general practices, community services and hospitals are seamlessly coordinated and incentivised to deliver patient centred, equitable care. This is important for both physical and mental health and should be offered from prevention through to end of life and in partnership with the individual, their carers and family.

Managing demand is complex and requires a whole of system approach that involves both the inpatient and outpatient elements of care. Areas that are likely to have impact include:

· The management of long-term conditions,

· Primary care hospital avoidance programmes,

· Provision of urgent access to primary care,

· Ambulance diversion pathways,

· Improving discharges and

· Understanding readmissions

To aid in overcoming these challenges we invite you to join us for the 2019 Integrated Care Program.

Participants are advised to bring their e-devices and free wi-fi available. Throughout the 2 days we will have staff available to assist you access and download your reports as well learn to use other services and features available to you such as AORTA so you can develop your own reports.

9.30 – 10.30am	Welcome and reflections from day one – MAKE IT HAPPEN- What can we learn from others – Innovations from practice relating to integration of care and services · Innovation poster sessions · Each service to highlight one or more innovation projects relating to service integration
10.30 – 11.00am	Morning Tea
11.00 – 11.30am	DECIDING WHAT TO DO – Blue sky thinking Working with your service team to clarify the burning platform issues that necessitate change in order to enable service integration · What is our data telling us, what have we learnt from the sessions, discussions, innovations shared? · Blue sky thinking ideation exercise to identify an area to address with a concept to explore
11.30 –12.00pm	IDEAS INTO ACTION – How to create long term change Walking through how an idea turns into an innovation that is implemented How to turn an idea into an innovation that can be implemented, adopted and becomes BAU. Teams use this as a Guide to identify push and pull factors that will help or hinder success
12.00 – 12.45pm	DECIDING WHAT TO DO – Planning practice improvements Forth working session: Group work on Action Plans · Aim statements and SMART Goals · Sharing and learning from improvements made by other member health services · “Sharing among peers” – checking out policies and procedures from our counterparts
12.45 – 1.30pm	Lunch
1.30 – 1.45pm	WINNING INNOVATION PRESENTATION
1.45 – 2.30pm	MAKING IT HAPPEN - Commitment Ceremony Attendee organisations present to the group their blue sky poster and Aim statement and seek support or help from others.
2.30 – 3.15pm	FEEDBACK ON THE REPORTS – On reflection Feedback to the HRT team on the 2 sets of reports: · How can we include Australian community health data? · Potential for current reports to benchmark integration initiatives
3.15 – 3.30pm	2020 meeting focus and closing comments Comments and suggestions on focus for the 2020 meeting and final comments from Health Roundtable General Manager. Evaluation of the workshop.
