The focus of the Chronic Care Improvement Group is to discuss appropriate care, including options for treatment and prevention activities outside of a hospital, for patients with chronic disease.

Overall, we know that patients with chronic disease do not suit an episodic model of care – which is generally more expensive and offers a fragmented patient experience. Radical overhaul of current care models, along with early intervention and coordination with Social Care, is required in Australia, New Zealand and the rest of the world as a ‘Chronic Disease Epidemic’ impacts on ageing populations. Many new models of care are being tried around the world.

SETTING THE STAGE – Identifying  the burning platform for change

Dr Amanda Landers – Nurse Maude, Canterbury NZ describes the challenges faced in the management of patients with chronic diseases and the benefits of redesigning more appropriate community services that meet the needs of this patient cohort. Amanda will discuss the application of palliative care in the community to patients with chronic illnesses and providing more appropriate support outside of the hospital. 

 This session will incorporate questions and small group / table discussion time

DECIDE WHAT TO DO

Tour of the Design Lab to show the innovative space where clinicians, engineers, architects and user groups are rethinking how health services are provided.

 ‘Innovations’ Poster session & Meeting Market Place - Each facility to highlight one or more innovation projects (preferably related to initiatives to understand and optimise the care of patients with chronic diseases).

 This session will include a break for Tea / Coffee

 Russell Harrison – Acting Chief Executive Officer Western Health, VIC will share his experience of the successes and lessons from an effort to develop a new model of funding for a large cohort of patients with chronic disease which started in October 2016

 This session will incorporate questions and small group / table discussion time

 – Deciding how transformational change can occur and developing a clear goal and vision of the change activities that are needed

(Small group discussion with other participants from same state or country on how issues can be addressed)

Not letting up – creating a new culture - Team Action Planning & Presentations

How to make change happen – people engagement, marketing & branding your change, risks to manage etc.

Teams to develop Action plans

All plans will be placed on wall posters for general improvement discussion / suggestions by other teams.

Finally, teams will refine their action plans.

Decide on theme for 2018 Readmission Improvement Group Workshop

Meeting Ends at 2:30pm to allow time to catch international flights.
