We would like to invite your health service to participate in our upcoming meeting on Improving Management of Patients with Chronic Conditions (with a specific focus on Diabetes), scheduled for 6th and 7th September 2016 at the Novotel Sydney Central.

The purpose of the meeting is to discuss appropriate care, including options for treatment and prevention outside of hospital, for patients with diabetes. Around 1.7 million Australians are known to have diabetes and this is rising by nearly 300 each day. Total annual cost impact of diabetes in Australia estimated at $14.6 billion. The prevalence in New Zealand is similar with over 300,000 having diabetes.2

The key objectives for this Roundtable are to:
• Compare your key data on hospital based diabetes treatment with Health Roundtable benchmarks,
• Identify practices of interest for care of diabetes patients at other member health services,
• Learn how to work with organisations outside the hospital to manage diabetes and intervene earlier in the journey to reduce the severity of cases / prevent serious conditions developing.

Presentations from Dr Sonia van Gessel (Director of Clinical Governance and GP Engagement Leader, South Eastern NSW PHN) and Professor Andrew Bonney (GP and Roberta Williams Chair of General Practice, University of Wollongong).

This session will include time for questions and small group discussions.

MAKE IT HAPPEN (3) - Turning issues into action

Small group discussion with other participants on how to:

  • Communicate for understanding and buy-in
  • Empower others to act
  • Produce short term wins

Further time to work on A3 action plans will also be available.

MAKE IT HAPPEN (4) – Don’t let up & create a new culture

A gallery walk of A3 action plans will start this session, followed by teams working on and refining their Aim statements and action plans.
