In 2014 the Mitchell Institute released a paper concluding the burden of chronic disease in Australia threatens to overwhelm the health budget, the capacity of health services and the health workforce.[1]  Primary Health Networks (PHNs) have been established with the key objectives of increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure patients receive the right care in the right place at the right time. 

The successful vendors for the PHNs were announced in April and will hit the ground running as of 1st July 2015. This special Roundtable has been convened to assist members and their associated PHNs to identify specific local opportunities in chronic disease management to focus on for improvement. 

The Health Roundtable will provide data to show how services are currently performing in managing chronic diseases in general, and specifically Heart Failure.  There will be presentations from the Federal Department of Health, from the acute care sector and PHNs on successful initiatives and lessons learnt in improving service integration. Depending on demand, we are looking to bring an international speaker into this meeting to provide an additional valuable perspective.  Delegates will have ample time for discussion, questions, networking and preliminary planning.

Who should participate in this meeting? 

Delegates are welcome from Health Roundtable member organisations, Primary Health Networks and others with an interest in this area.  Members of the Health Roundtable should send an Executive Sponsor, a clinical leader, your chronic disease program manager and make sure you invite a senior representative from your local PHN.
