In 2014 we are holding a special Roundtable to improve how hospitals and primary care organisations work together to provide coordinated care for patients with COPD and other chronic conditions.

Many members in New Zealand and Australia are already working closely with primary care organisations, but recognise that more needs to be done to improve patient care. This meeting will help you to compare your practices with other leading organisations and to identify further improvements. Click here for brochure

Thursday 14^th August 2014
8:30 - 9:00	Registration
9:00 – 9:15	Introduction at tables and introduction to The Health Roundtable.
9:15 – 10:30	Comparison of your results on COPD (including Geo-mapping of Emergency Presentations, Relative Stay Index and Readmission Rates). Review of responses and trends from Survey. Session will incorporate group discussion time.
10:30 -11:00	Morning Tea
11:00 – 12:00	KEYNOTE PRESENTATION: Key lessons from England on COPD Strategy Professor Sue Hill will talk about learnings from the implementation of the National Clinical Strategy for COPD in England which was a network approach involving professionals and managers across primary, secondary and tertiary care.
12:00—12:30	Good Practice in 2020 – Small Group Discussion What actions are needed to reduce readmissions in COPD?
12:30 -13:30	Lunch
13:30 – 15:00	Poster viewing session – Sharing innovations on COPD Management Each organisation to highlight one innovation around working with Primary Care. The group will identify posters and topics for in-depth coverage.
15:00 — 15:30	Afternoon Tea
15:30 – 16:00	Hospital Team Huddle: What have we learnt? Hospital teams have time to discuss and consolidate learning from the day.
16:00	Day One concludes

9:00 – 9:20	Recap of Day One
09:20 – 10:00	The view from Primary Care – Thought Starter presentation This presentation will assist member hospitals in seeing the view from Primary Care and assist in understanding how this relationship can be further developed.
10:00 – 10:30	Small group discussion around key issues
10:30 -11:00	Morning Tea
11:00 – 11:45	Hot topics session Facilitated session – sharing ideas, innovations and best practice on hot topics. Helping your colleagues address key issues on working with primary care.
11:45 – 12.30	Action Planning & Next Steps We will provide coaching to help you develop a one-page “business plan” to improve how you will work with primary care colleagues based on this meeting.
12:30—13:30	Meeting Concludes with Lunch
13:30 – 14:30 [Optional Session]	Optional Health Roundtable Data Review Optional session looking at data available from the Health Roundtable including online reporting and Geo-mapping tools.
