 The End of Life Care Group will hold its third annual meeting in Melbourne on 19-20 June 2014, with a focus on identifying patients at the end of life. Click here for information brouchure

Member organisations are encouraged to bring along your community and primary care partners to this meeting, as we share practices and innovations on predictive tools as well as how to deliver and fund end of life care. 
 
The Health Roundtable will analyse inpatient data to review the variation in admission rates to hospital in the last six months of life for illnesses such as cancer, COPD and heart failure, and variation in ICU admissions in the last month of life.
 
Delegate Registrations may be cancelled up to 7 days prior to the commencement of the meeting, with a full credit of monies paid. After that date, no credits are possible. However, you may substitute a different person for those registered at any time. Hotel and airfare arrangements are separate, and may have different policies regarding cancellations and name changes. Please contact them directly.

Challenges for Funding End of Life Care: This workshop will explore the impact of the new funding models in Australia particularly the challenge of providing and funding specialist palliative care, community and consult services. Key contributors will include Assoc. Professor Rohan Vora, Director of Palliative Care Services, Gold Coast Health Service and Chris O’Gorman a consultant with The Health Roundtable who has extensive knowledge of the implications of the ABF model. Cost and revenue data will be available for hospitals with costing data for 2012/13 for palliative care episodes.
