Chronic kidney disease is a leading public health problem. Every day, six new patients start dialysis with numbers expected to double by 2020.
This improvement workshop is being convened by The Health Roundtable with participation by Kidney Health Australia and The National Renal Clinical Directors Network.
The aim of this workshop is to share practical innovations to improve the management of patients with end-stage renal failure, including:
Dialysis modalities and complications
Transplantation
Criteria for starting/stopping dialysis
End of life management
Casemix funding – lessons learnt and best practices
The format is inter-disciplinary teams and interactive small groups to maximise sharing of knowledge and networking opportunities. Delegates will use data to identify exemplars and share good ideas to develop an improvement plan over the two-day meeting.
Renal services are encouraged to share data and innovations by:
· Comparing your care systems and results: A snapshot of clcnial indicators
· Benchmarking through inpatient data reports on a range of indicators, such as readmission rates, LOS and others to be determined by member survey
· Sharing your good ideas! This is a unique forum of networking where innovations on all aspects of a renal service can be shared. Ie: Apps, training materials, role development, patient support and education programs, clinical pathways, primary care collaboration models
The meeting concludes with an optional state-of-the-art renal system modelling workshop to demonstrate the best and only practical options to manage the many complex variables. Find out how system modelling could improve practical service planning and day-to-day management. These capabilities will be demonstrated by examples of simulation modelling used successfully to improve renal services (see basic example http://www.synergia.co.nz/what-we-do/modelling-and-simulation) then develop consensus specifications for now needed new models.
