Delegates to meet at 1.45 level 2  ( Radiation Oncology) Olivia Newton John Cancer Centre

This workshop will focus on long term follow-up solutions for prostate and breast cancer patients. By collaborating with colleagues, practical ways to improve patient’s transition from the acute treatment setting into follow-up will be shared.  The group will hear about initiatives that showcase who, what and where alternatives for follow-up have been implemented.  The aims are:

 
·To increase the efficacy and productivity of the oncology workforce in acute facilities;
·Increase new patient access to oncology services
·Reduce variation in follow-up protocols, both clinical and psychosocial, to improve patient outcomes
· Determine the most appropriate personnel and settings for follow-up, including Wellness Centres
· Determine the business case ‘Wellness Centres”

There are two components to the fee structure for this Roundtable.  
The organisational fee per meeting is $5,000, which includes the cost of analysing the attached survey and facilitating the meeting.  If your service model is a network, as in the Victorian model, additional services can participate for $3,000 each. 
The delegate fee of $230 + GST per person covers the cost of the hotel meeting facilities and refreshments for the two days.

Delegate Registrations may be cancelled up to 29 August 2013, with a full credit of monies paid.  After that date, no credits are possible. However, you may substitute a different person for those registered at any time.  Hotel and airfare arrangements are separate, and may have different policies regarding cancellations and name changes.  Please contact them directly.

 Accommodation & Travel/Flight Arrangements

Delegates are responsible for making their own accommodation and travel/flight arrangements.

The Novotel Melbourne on Collins has set aside rooms for the nights of 4 & 5 September 2013 for The Health Roundtable as follows:
