Delegate Registrations may be cancelled up to 8 May 2013, with a full credit of monies paid.  After that date, no credits are possible. However, you may substitute a different person for those registered at any time.  Hotel and airfare arrangements are separate, and may have different policies regarding cancellations and name changes.  Please contact them directly.

 

Note: Delegates are responsible for making their own accommodation and travel/flight arrangements. If you require accommodation you must book before 14 April 2013.  Delegates are responsible for making their own accommodation and travel/flight arrangements.

The Amora Hotel Wellington has set aside rooms for the nights of 14 & 15 May 2013 for The Health Roundtable at the Conference rate of NZ$195.00 (Deluxe Room) – Room only including GST.

Delegates should complete the Accommodation Booking Form and forward to the Reservations Department to receive the conference rate as follows:
