Delegate Registrations may be cancelled up to 3 November 2011, with a full credit of monies paid. After that date, no credits are possible. However, you may substitute a different person for those registered at any time. Hotel/Venue and airfare arrangements are separate, and may have different policies regarding cancellations and name changes. Please contact them directly.

9.15 – 10.30am	Keynote speaker – Dr Ken Catchpole “Using Prospective Hazard Assessment to Enhance Safety and Quality in Health Care”
10.30 – 11.00am	Morning tea
11.00 – 12.30pm	Review of Operating Theatre data collected by members & what does this mean for your hospital? Workshop session in small groups to brainstorm implications of data results
12.30 –1.00pm	Lunch
1.00 – 2.30pm	Project progress feedback (This session will provide an opportunity for all participants to report on progress with their project since the last meeting.)
2.30 – 3.00pm	Hot Issues Forum: Develop list of key issues affecting your Health Service for the next session Get input from other teams on your key issues
3.00 – 3.30pm	Afternoon tea
3.30 – 4.00	Team Action planning What good ideas have we heard and what will you do to improve theatres in your Health Service?
4.00 – 4.15	Discussion on next steps and maintaining momentum
4.30pm	Meeting concludes

Accommodation & Travel/Flight Arrangements

Delegates are responsible for making their own accommodation and travel/flight arrangements.

The Novotel Brighton Beach has set aside rooms for the nights of 8 and 9 November 2011 for The Health Roundtable at the Conference rate of AUD$229.00 (Standard King) – room only including GST.
