Delegate Registration Instructions - Please Register by 4 May 2008

Please select the specific sessions you plan to attend and tick only 1 SESSION in each time slot.

Please also tick Package 13 - Group Dinner - Wednesday 4 June if you will be attending. Venue to be advised.

Package 14 - Presenter's Discount: There is a discount of $100 for one presenter discount per registered participant.

To qualify for the Presenter's Discount, please send an email TODAY to [email protected] with the proposed Title of the Presentation and the session number of the Workshop for which you would like the Presentation considered. We will contact you to discuss the Presentation.

If you require accommodation at the Hotel, please make your reservations by 4 May 2008.

Delegate Registrations may be cancelled up to 28 May 2008, with a full credit of monies paid. After that date, no credits are possible. However, you may substitute a different person for those registered at any time. Hotel and airfare arrangements are separate, and may have different policies regarding cancellations and name changes. Please contact them directly.

Delegates are responsible for making their own accommodation and travel/flight arrangements.
 
The Stamford Plaza Double Bay has set aside rooms for the nights of 3 & 4 June 2008 for The Health Roundtable at the Conference rate of AUD$205.00 (room only including GST).
