NOTE: Dr Raj Behal, will also provide an optional half-day workshop on the morning of 21 August at the Sofitel prior to the start of the Roundtable for those unable to attend any of the other workshops. When registering please select the Workshop option if you would like to attend.

If you require accommodation at the Sofitel Gold Coast, please print out and complete the Accommodation Booking form and return it to the hotel by 20 July 2008.

Delegate Registrations may be cancelled up to 14 August 2008, with a full credit of monies paid. After that date, no credits are possible. However, you may substitute a different person for those registered at any time.

2.00 – 3.00pm	PowerPoint Poster Session: Key Innovations in Patient Safety Participants share most important safety improvement in the past 12 months (including afternoon tea).
3.00 – 4.00pm	Specific Poster Discussions: Posters selected by participants will be discussed in depth in small groups.
4.00 – 4.30pm	Hospital Team Feedback Session: What good practice ideas to explore?

9.00 – 10.30am	Sentinel Event Poster Session: Participants share examples of “closing the loop” implementing preventive actions following a specific sentinel event of relevance to other members
10.30 – 12.00 noon	Hot Issue Forum: What one key issue would your delegation like to get input on from the other participants? (including morning tea).
12.00 – 12.45pm	Hospital Team Workshop: Reviewing feedback on your “hot issues” and testing feasibility of innovative ideas to take back for implementation.
12.45 – 1.00pm	Next Steps: Collaborating on New Approaches for Sustainable Improvements in Patient Safety Systems and Procedures.
1.00 – 2.00pm	Lunch
2.00pm	Roundtable concludes.

Accommodation & Flight Arrangements

Delegates are responsible for making their own accommodation and travel/flight arrangements.

The Sofitel Gold Coast has set aside rooms for the nights of 20 & 21 August 2008 for The Health Roundtable at the Conference rate of AUD$230.00 (room only including GST).

To receive this rate, Delegates are required to complete and return the Accommodation Booking form to the Hotel.

To reserve one of these rooms please book before 20 July 2008.

Requests for accommodation after this date will depend on the Hotel’s availability.

The Gold Coast is easily accessed by domestic and international guests via Coolangatta Airport - 20 kms away 25 mins by taxi from Broadbeach (Virgin now has more passengers in/out than Qantas).

Brisbane International Airport is an easy 1 hours drive.
